Background:

This program contributes to the Interprofessional Primary Care Team (IPCT) project. It is a primary care initiative, aiming to increase access and attachment to interprofessional, team-based primary care for individuals in Downtown East Toronto experiencing significant financial and social barriers. The project places a specific emphasis on addressing the unique needs of Indigenous and Black communities, particularly those who are experiencing housing insecurity, identify as part of the 2SLGBTQ+ community, are refugees, living with disabilities, or use substances.
